General Questions
How do I make an appointment?
If you would like to make an appointment, please call our office at (302) 204-1639 or submit a request on our website. Patients will receive a reminder call the 2 business day and a text reminder 1 business day prior to the appointment.
What do I bring to my appointment?
1. Past medical records (if available).
2. All current medications (including over-the-counter)
3. Insurance: Should be brought with every appointment.
4. ID: Current state ID or driver license.
What insurance do you accept?
We are currently in-network with all Aetna plan, Medicare and DE Medicaid.
Our office is working hard to enroll in all major health insurance. Please contact the office for most up to date in-network health plan.
For all questions regarding your insurance benefits, please contact your health plan directly.
What is your policy for payments?
Payments or any prior balance is due at the time services are rendered. You are responsible for your deductible, co-payment and co-insurance obligation.
Payment plan is available and must be arrange prior to services are rendered.
What is your no show/cancellation policy?
No-Show and Last Minute Cancellation Policy
At PRIMECARE Comprehensive Health, LLC, we strive to provide the highest level of care and service to all of our clients. Our appointment-based system is designed to ensure that everyone has access to the services they need in a timely manner.
No-Show Policy
A “no-show” is defined as a missed appointment without prior notice. When patients do not attend their scheduled appointments, it affects not only their health but also other patients who may have been in need of those time slots. As a standard practice, we ask clients to notify us at least 24 hours in advance if they are unable to make their appointment.
Last Minute Cancellation Policy
We understand that life can be unpredictable, and sometimes circumstances arise that prevent clients from keeping their appointments. As a courtesy to our patient, we will waived the first last minute cancellation fee.
Last Minute Cancellation Definition: A last-minute cancellation is defined as a cancellation made within 24 hours of the scheduled appointment.
Cancellation Process:
- Clients must notify us via call, text, or email as soon as they realize they cannot make their appointment.
No show/last minute cancellation fee:
To maintain this standard, we have implemented a No-Show and Last Minute Cancellation fee. This policy helps us manage our schedule effectively while also accommodating individuals who are in urgent need of services.
- There is a $ 45 fee charged for regular appointments and a $70 fee charged for extended appointments (Physical, form filling, etc) to patients who fail to cancel their appointment without giving a 24-hour notice. These fees are not covered by insurance and is solely the responsibility of the patient.
- There is a $65 fee charged for no call/no show for regular appointments and a $85 fee charged for extended appointments (Physical, form filling, etc).
- Repeated no-shows of 3 missed appointments, will result in discharge of the patient from the practice.
Fees are not covered by insurance and will be billed directly to you.
What should I do if I am are running late?
Please arrive to your appointment 15 minutes early. If you are running late, please call the office as early as possible. Appointment will be hold for a grace period of 15 minutes. If you need to run later than that, we may need to ask you to reschedule your appoinment (this will consider a last minute cancellation) or we may push your appointment to a later time on the same day to avoid the last minute cancellation fee.
What if I need a copy of my medical records?
Your medical records are protected by the Health Insurance Portability and Accountability Act (HIPAA). This means you have the right to request that we restrict how your Protected Health Information (PHI) is used or disclosed. According to HIPAA guidelines, you must sign a release of information form before we can release your medical records.
Our fee for copying medical records is: A $10 processing fee plus $0.50 per page (up to 50 pages). Additional pages (after 50) are $0.25 plus postage. We require a one week notice to process your request.
What if I need a referral?
Please contact us for all referral requests. Any patient that have been seen within the last 3 months may not require a follow-up visit for referral needs. Please allow 5 days for all referral requests.
What if I needs form to be filled out?
PRIMECARE Comprehensive Health, LLC will accommodate as many requests as possible in an accurate and timely manner. Blank forms will not be accepted. Personal information must be completed. Turnaround time for forms and letters are 5 business days.
Forms are only completed for those accounts that are in good standing. Outstanding balances need to be paid before forms are filled out.
FORM FEES
- Physical form for school/work presented during visit are at no charge.
- Physical form requested after visit are subject to $25 fee.
- Reviewing and completing medical forms is $45.
- Medical letter are $20 each.
Do you fill out FMLA form?
We do FMLA form for our established patient of at least 6 months. We do not fill out FMLA form for new patient since we have no prior medical history to fill out on.
